2023 Hy-Vee Fundraising Cards Order Form
The 2023 Hy-Vee Fundraising Cards are now available! They are a great fundraising opportunity for your respective school, church, organization or team. This order form must be submitted at least 48 hours in advance of your desired pick up date.

Please note that all orders are filled out of the Coralville Hy-Vee and Oakland Road Hy-Vee locations, whichever store is most convenient for you. Payment is due upon pick-up. A confirmation will be sent once your order is ready for pick up.
