ECG Challenge: In the ED With Palpitations and Shortness of Breath

A 67-year-old man with a history of hypertension and mild mitral regurgitation (treated with a beta-blocker and hydrochlorothiazide) presents to the emergency department because of intermittent palpitations and shortness of breath. His physical examination reveals a slow pulse and a soft holosystolic murmur but is otherwise unremarkable.

An ECG is obtained.
