Antidepressants work via one or more mechanisms of action to block the active reuptake of norepinephrine, serotonin, and dopamine. They have both anticholinergic and sedative effects. The choice and dose of medication are guided by estimated safety and tolerability, physician familiarity, and a history of prior treatment. A clinical response with a therapeutic dose can be observed within 2-12 weeks of therapy induction, depending upon several factors, such as type of treatment, underlying health, and other mood disorders. Treatment failure in depression is most often linked to medication noncompliance or inadequate dosing/duration of therapy, although a modification of therapy is sometimes necessary for patients who are treatment-resistant.
