ECG Challenge: What Caused This Man's Atrial Arrhythmia?

A 77-year-old man with a history of hypertension, treated with an angiotensin-converting-enzyme (ACE) inhibitor and beta-blocker, has a long history of atrial fibrillation with adequate rate control. He presents to his primary care provider for a routine physical examination, and a 12-lead ECG is obtained.
